Introduction
Current guidelines for the management of valvular heart disease serve as an essential guide for clinicians. The ESC 2025 Guidelines on Valvular Heart Disease make clinical decision-making more systematic and practical with their updated recommendations. This bulletin provides a summary of the revised recommendations from the guideline.

Commentary:
The revised recommendations in the 2025 ESC Guidelines on Valvular Heart Disease emphasize patient-centered, individualized, and long-term management strategies. The Heart Team approach is now recommended not only for complex cases but also for the comprehensive evaluation of all valvular diseases. For TAVI candidates with concomitant coronary artery disease, non-invasive imaging—such as coronary CT angiography—is preferred over invasive methods in low-to-intermediate risk groups. In native valve disease, rhythm control in atrial fibrillation has regained importance; during mitral valve surgery, concomitant surgical ablation and left atrial appendage closure are now more strongly recommended. Another key update concerns DOAC use in rheumatic mitral stenosis, where the contraindication threshold has been redefined: DOACs are contraindicated in patients with a mitral valve area ≤2 cm², while individualized decisions are advised for those with larger valve areas. In mitral and tricuspid regurgitation, early diagnosis and timely intervention are emphasized, with a stronger recommendation for transcatheter mitral interventions (especially TEER) in patients with suitable anatomy and moderate-to-high surgical risk. Overall, the guideline promotes a holistic, multidisciplinary approach aimed at improving long-term outcomes through structured, collaborative clinical decision-making.
