Introduction:
ACURATE neo2 is a new generation self expanding transcatheter aortic valve. In the ACURATE neo2 valve vs other balloon-expandable (SAPIEN3/ULTRA) and self expanding (Evolut R/PRO/PRO+/FX) valves trial, ACURATE neo2 did not meet non-inferiority criteria when compared to the other aortic valves.

Objective:
To evaluate impact of ACURATE neo2 valve under expansion and clinical outcomes.

Methods:
Post hoc analysis of patients who are implanted ACURATE neo2 valves, in some patients there was angulated commissure posts which indicates valve under expansion. In the trial, 752 patient were implanted ACURATE neo2 valves. All these patient were evaluated with angiogram by indepedent core laboratory. Main aim in these study to evaluate association between valve expansion and clinical outcomes.

Results:
In this trial, 624 patient who were implanted with ACURATE neo2 were available for angiogram.135 patient (21.6%) had under expanded valves. Independent cause of under expansion of valve is associated with greater aortic valve leaflet and annulus calcification. There was no relation with under expansion of valves in procedural techniques such as frequency of predilation (100% in both groups) and postdilation (26.7% in under expanded group and 25.2% in expanded group) and balloon sizing. As a result, under expanded valves was associated with a higher 1 year rate of death, stroke, or rehospitalization (under expanded 18.7%, expanded 11.8%).

Conclusions:
These trial shows us under expanded ACURATE neo2 valves has higher rate of 1 year death, stroke and hospitalization. This under-expansion is more likely to occur in patient with greater aortic leaflet and calcific aortic annulus.

Commentary:
This study clearly demonstrates that in TAVI procedures, not only the appropriate valve selection but also optimal positioning and full expansion of the valve are critical determinants of clinical outcomes. These findings important for analyses in guiding future device design and development.
