Aims
The STEEER-AF trial was designed to outline the rationale, design, implementation, and baseline characteristics of the Stroke Prevention and Rhythm Control Treatment: Evaluation of an Educational Programme by the European Society of Cardiology. The primary goal was to objectively assess adherence to guidelines in routine clinical practice and to evaluate the impact of a targeted educational program for healthcare professionals.

Methods and Results

STEEER-AF is a pragmatic trial aimed at rigorously determining guideline adherence and assessing the effectiveness of an educational intervention for healthcare providers. The trial involved randomization of 70 centers across six countries (France, Germany, Italy, Poland, Spain, and the UK) between 2022 and 2023. A total of 1732 patients with atrial fibrillation (AF) were recruited, with a mean age of 68.9 years (SD 11.7) and a CHA2DS2-VASc score of 3.2 (SD 1.8); 647 of these patients (37%) were women. At enrollment, 843 patients (49%) were in AF, and 760 patients (44%) were in sinus rhythm. Oral anticoagulant therapy was prescribed to 1543 patients (89%), with the majority receiving direct oral anticoagulants (1378; 89%). Previous interventions such as cardioversion, antiarrhythmic drug therapy, or ablation were noted in 836 patients (48.3%). Currently, 551 patients (31.8%) were on antiarrhythmic drugs, and 446 patients (25.8%) were scheduled for future cardioversion or ablation. The educational program involved 195 healthcare professionals from centers randomized to the intervention group, including bespoke interactive online learning and reinforcement activities, supported by national expert trainers.

Conclusion

The STEEER-AF trial was successfully conducted across six European countries to explore guideline adherence in real-world settings and to evaluate whether a structured educational program for healthcare professionals can enhance patient-level care.

Comment
The STEEER-AF trial demonstrates that targeted education for health care professionals can improve patient-level guideline adherence where there are substantial gaps in implementation, as clearly demonstrated for rhythm control in AF.
