Three-Year Outcomes After Transcatheter or Surgical Aortic Valve Replacement in Low-Risk Patients with Aortic Stenosis

Objective:
Randomized data comparing the outcomes of transcatheter aortic valve replacement (TAVR) with surgery after 2 years in patients at low surgical risk are limited, and this study aimed to evaluate the 3-year clinical and echocardiographic outcomes of the “Evolut Low Risk” trial.

Methods:
Patients were randomized to TAVR and surgical intervention groups and the primary endpoint was all-cause mortality or stroke with sequelae.

Results:
The study enrolled 1414 patients (730 TAVR; 684 surgery). The mean age of patients was 74 years and 35% were women. The primary endpoint occurred in 7.4% of TAVR patients and 10.4% of surgical patients (HR, 0.70; 95% CI, 0.49-1.00; p=0.051). The frequency of mild paravalvular regurgitation (20.3% TAVR, 2.5% surgery) and pacemaker implantation (23.2% TAVR, 9.1% surgery; p<0.001) was lower in the surgical group. Moderate or greater paravalvular regurgitation was less than 1% in both groups and did not differ significantly. Valve function improved significantly at 3 years in patients who underwent TAVR (mean gradient 9.1 mmHg TAVR, 12.1 mmHg surgery; p<0.001).

Discussion:
Overall, in the “Evolut Low Risk” trial, TAVR was more successful than surgery in terms of all-cause mortality or stroke with sequelae at 3 years.
