Introduction:
Calcified coronary lesions are common and can impair stenting outcomes. ICARE directly compares intravascular lithotripsy (IVL) and rotational atherectomy (RA) for plaque modification prior to stent placement.

Objective:
To assess whether IVL is non-inferior to RA regarding minimal stent area (MSA) and compare safety profiles.

Methods:

  • Design: Prospective, multicenter, randomized non-inferiority trial
  • Participants: 202 screened, 170 randomized (RA: 86, IVL: 83)
  • Imaging: Optical Frequency Domain Imaging (OFDI)
  • Primary endpoints: Acute MSA (efficacy), 30-day MACE (safety)

Results:

  • MSA: RA = 5.5 mm²; IVL = 5.8 mm² (non-inferior, p<0.05)
  • MSA <4.5 mm²: Similar (RA: 21%, IVL: 22%)
  • Malapposition higher in RA (80% vs 61%, p=0.006)
  • IVL had lower fluoroscopy time and contrast use
  • Periprocedural MI: more frequent in RA (5.8% vs 0%, p=0.06)
  • 30-day MACE: no significant difference; no early deaths or stent thrombosis

Conclusion:
IVL is non-inferior to RA in terms of efficacy and comparable in safety. It offers procedural advantages with less malapposition and resource use.

Comment:
IVL emerges as a safer, less invasive alternative to RA for calcified lesions, supporting broader clinical use pending long-term data.
